You'll find the most common credit union applications, forms, and disclosures on this page.  All applications and forms listed on this page allow you to input information right from your computer.  Please review description attached to each application/form for more information.  

Please do not email any form to us if it contains your personal information.

The Quick Links give you direct access to manage your account, pay a bill, and access credit card information (MyCardInfo).  You may also use the "Sign Up" links to start registering for these free, easy, and secure services.

Checking Account Application
To help expedite the account opening process and for your convenience, you may fill out any Account Application listed on this page if you are interested in opening an account with VADPFCU.  However, the account opening process may only be complete IN-PERSON.  Please bring your complete application(s) along with necessary documentation (see FAQ for more information) to one of our branches so we may help you set up your account.

Loan Application
If you are an existing member, you may use our online application process to apply for your loan. If you are eligible for membership and interested in one of our loan products, you may also complete the online application. However, before the loan can be funded, you must set up your membership with VADPFCU.  Please visit How to Join for more information about membership eligibility and benefits.

Savings Account Application
To help expedite the account opening process and for your convenience, you may fill out any Account Application listed on this page if you are interested in opening an account with VADPFCU.  However, the account opening process may only be complete IN-PERSON.  Please bring your complete application(s) along with necessary documentation (see FAQ for more information) to one of our branches so we may help you set up your account.

ATM/Debit Card Application
Whether you are looking to order a new ATM or debit card or replace a lost or damaged card, you can use this electronic application to complete your request. Remember that if you are looking to set your own personalized Personal Identification Number (PIN), you must visit your local branch and present your card and a form of valid identificaiton.

Change of Address/ Contact Information
Accurate and current mailing/contact information is essential to assist us in protecting your accounts and notifying you about relevant changes.  

You may also request a change of address by phone.  Please see the FAQ page for more information.

Joint Account Owner/ Beneficiary Update
You may use this form to add a new Joint Account Owner, and/or add or update the beneficiary on your account.  To complete the process, please bring the Joint Account Owner, this complete form, along with necessary documentation (see FAQ for more information) to one of our branches so we may add a Joint Owner to your account.

To add or update a beneficiary on your account, you may fax, mail, or drop off the complete form at any of our branches for processing.  Please be sure to include all identification information of the beneficiary for ease of future reference and verification.  

Incoming Wire Transfer Instruction
This form provides all necessary information you need to send funds from your account at another financial institution to your account or loan at VADPFCU.

Outgoing Wire Transfer Instruction
This form requests specific information in order for us to send your funds to another financial institution, domestic or foreign.  Wire Transfer is a safe and quick method you may use to transfer funds.  You may fax or drop off this form at any of our branches for processing.

Trust Account Agreement
This form is necessary in order for VADPFCU to set up a Trust Account, to change an existing Individual Account to a Trust Account, or to update the Trustor, Trustee, or Beneficiary information in your Trust Account with us.  We do not need a copy of your Trust document.  We only need this Trust Account Agreement to make changes.  You may fax, mail, or drop off this form at any of our branches for processing.  

Disclosures (In Alphabetical Order)

EFT Disclosure
This Disclosure is made in compliance with federal law regulating electronic funds transfer (EFT) services. Electronic funds transfers are electronically initiated transfers of money involving an account at the credit union. The following disclosures set forth your and our rights and responsibilities concerning the electronic funds transfers.

E-Statement Disclosure
By accepting the E-Statement Disclosure, you consent and agree that the credit union may provide certain disclosures and notices to you in electronic form, in lieu of paper form, including electronic delivery of statements (e-Statements) for your credit union accounts.

Your Insured Funds
Your deposits in the credit union are insured by the National Credit Union Share Insurance Fund (NCUSIF), which is back by the full faith and credit of the United States Government.  Established by Congress in 1970 to insure member share accounts at federally insured credit unions, the NCUSIF is managed by NCUA under the direction of the three-person NCUA Board.

Truth-in-Saving Disclosure
Also known as "About Your Credit Union Accounts".  The disclosure describes all of the features, restrictions, terms and conditions for all credit union's deposit accounts, as required by laws.  The disclosure may assist you in making meaningful comparison between products, rates, and fees among competing financial institutions.
